This questionnaire will help you check the readiness of an organization for computerization of activities. Note :-- The persons holding managerial positions at higher levels need to answer these questions for judging the readiness of an organization.

]   What is your present mode of functioning -- manual record keeping or computerized MIS ?

-a]   Does your present mode cover all your MIS needs ?
-b]   Is there anything that is being done but has become redundant and needs to be discontinued ?
-c]   Is there anything that is not being done but needs to be included in the new MIS mode ?

]   Are your present procedures being followed rigidly or are there any lapses, by-passing etc. ?

]   Are your present practices stabilized or do you need to make frequent changes in procedures ?

]   If it is needed or found feasible, are you willing to change your present procedures or norms to reduce the complexity of developing the new MIS ?

]   Can you ensure that, under the new computerized MIS, all procedures will be followed rigidly ?

]   Can you devote your sufficiently long time for discussing, understanding and verifying the intricacies at various stages in development of new MIS ?

]   Can you spare various operating level personnel so that they also devote their sufficiently long time for discussing, understanding and verifying the intricacies at various stages in development of new MIS ?

]   Do you accept the fact that 'the flexibility available to human mind and human brain is not available to machines & computer based MIS' ? Please note that the limitations of all machines impose certain limitations on our functioning and we need to fully comply with well defined procedures for success of computerized MIS.

]   Can you ensure that yourself, your management team members and various other employees will be available for training on new MIS, regardless of present work load ?

]   Can you ensure that all the users of new MIS are already conversant with elementary use of computers viz. starting the OS, menus & command buttons, login, logout, shutdown etc. ?

]   Although we have to keep the costs to minimum possible level, cheap solutions do not necessarily mean cost reduction. On the contrary, cheap solutions may actually turn out to be costly or useless in the long run. Thus, sufficient additional investment in the beginning may provide better & cost effective solutions in the long run. Do you accept this fact and are you willing for the same ?
